Please Click (download) and print, and fill out the Client Contract Form and the Confidential Client Information Form (LBL Client Intake) and return them to:  Lauren Pohn, P.O. Box 103, Delavan, WI 53115, along with a current photo and your deposit.

Read The Information to Bring to Session and make sure you bring a double-spaced copy of this information to your first session.  If you need a Health Care Provider's Release, please print it and bring to your health care provider who will then send it on to me.
