Read The "Cast Of Characters" directions and make sure you send a double-spaced copy of this information with the above forms. You can continue to make changes to this form even during your session. If you need a Health Care Provider's Release (you need this if you are currently seeing a psychotherapist for an issue for which you are taking prescribed medication), please print it and bring to your health care provider who will then send it on to me.
